The following is an updated explanation of the various deductions on your pay stubs.

1. RELIASTAR LIFE INSURANCE (LSA INSURANCE) – This insurance is voluntary and serviced by the LSA. LSA office phone number is (313) 961-5699.
CODE: 40700 (single) $14.68 bi-weekly.
CODE: 40702 (family) $19.65 bi-weekly.
A. $50,000 to beneficiary upon death of member.
B. $12,500 added to $25,000 for accidental death or dismemberment.
C. $10,000 coverage on dependents (spouse and dependent children 6 months to 23rd birthday).
D. $500 coverage for children 15 days to 6 months old.
E. Plan may be continued after retirement.
F. After age 65 the coverage and premiums are reduced for members. Dependents remain the same.

RETIRED MEMBERS:
CODE: 40742. Retiree w/o dependent, over 65. $10,000. $15.21 a month.
CODE: 40747. Retiree w/o dependents, over 65. $12,500. $19.00 a month.
CODE: 40746. Retiree w/dependents, over 65. $12,500. $31.21 a month.
CODE: 40743. Retiree w/o dependent, $20,000. $31.11 a month.
CODE: 40745. Code refers to retiree w/dependent. $20,000. $43.32 a month.

2. HARTFORD LIFE INSURANCE (DPOA INSURANCE) – DPOA Office, 313-567-8770, Change Beneficiary.
CODE: 40652. Family. $11.74 bi-weekly.
CODE: 40650. Single. $9.75 bi-weekly.
A. $25,000 payable to beneficiary upon death.
B. $35,000 payable to beneficiary upon death, if accidental.
C. $10,000 coverage for dependents: Spouse and dependent children from 6 months to 23 years of age.
D. $500 coverage for dependent children 14 days to 6 months of age. (Plan can be continued after retirement, with payment coming from holder’s retirement fund. After age 65 premium and coverage cut in half.)

RETIRED MEMBERS:
CODE: 40610. Single. $19.56 monthly under age 65.
CODE: 40620. Married. $23.02 monthly.
CODE: 40630. Survivors benefit, widow can keep $5,000 coverage for $3.43 a month.
CODE: 40640. Single. $9.77 monthly over age 65.
CODE: 40650. Dependent coverage 0/65 years. $12.40

3. DETROIT POLICE BENEFIT & PROTECTIVE ASSOCIATION – To check on/change beneficiary – New Center One, 3031 W. Grand Blvd. Ste. 405, (313) 870-9301, (313) 870-9330 fax
Cost: $8.56 (40620 active) bi-weekly; $18.55 (40315 retired) monthly.
A. $500 – $4,000 paid to member for duty or non-duty disability retirement..
B. $15,000 death benefit paid to beneficiary less any return of assessments.
C. $7,500 additional death benefit if determined in the line of duty by the department.
D. All assessments paid to member on 25th year as a member, less disability return of assessments.
E. Policy paid in full after 40 years as a member.

4. MEDICAL CODE

5. EMPLOYEE BENEFIT PLAN – While active this plan is mandatory and serviced through the Pension Bureau, Room 908, City County Bldg., 313-224- 3362. (Listed as Ben Plan on pay stub)
A. $4,900 – Police Officers – $.40 per pay.
B. $10,000 – Lt’s & Sgt’s – $.50 per pay .
C. Plan may be continued after retirement at (.09 cents/month). Coverage is determined by your length of service.

6. DETROIT POLICE OFFICERS DEATH BENEFIT PLAN – The insurance is voluntary and serviced by the DPOA, 313-567-8770.
ACTIVE MEMBERS – CODE: 40630. $2. Deducted only when an active member who belongs to the plan dies on or off duty. Total amount of policy depends on number of participating members.
RETIRED MEMBERS – A $20 deposit is paid to the DPOA at the time of Disability Retirement. $2 deductions are taken from the deposit until exhausted at which time another statement will be sent to the member requesting another deposit.
