Extended Medical Open Enrollment and Election Dates{0}

Due to the granted extension of Medical Open Enrollment until Sunday, November 30, 2014 COPS TRUST Representative Lisa Duncan will be in the DPLSA Office located at 28 W. Adams, Suite 700 (Woodward and Adams) on Tuesday, November 25, 2014 from 10 am to 2 pm all DPD members can sign up for Cops Trust and the Cops Trust VSP Optical Vision Plan.
Further, representatives from all Health care providers will be at Police Headquarters tomorrow, Friday, November 21, 2014 from 8 am to 11 am on the 6th Floor – Police Personnel.
If you will be signing up for the VSP, please don’t forget to bring a check and/or money order in the amount of $150.00 for the VSP Optical.
