If you have dependents enrolled on the City’s health care plan, you should have received a mailing from Dependent Verification Center.
DO NOT DISREGARD THIS MAILING! RESPONSES ARE MANDATORY AND MUST BE SUBMITTED ON OR BEFORE THE DEADLINE DATE REFERENCED IN YOUR LETTER TO PREVENT REMOVAL OF YOUR DEPENDENT (SPOUSE/CHILD(REN)) FROM THE CITY’S PLANS.
If you have a dependent on your plan and have not received your packet, CALL the Dependent Verification Center at 1.800.725.5810.
NOTE: This mailing is ONLY for employees who have dependents (spouse or child(ren)) enrolled on the City’s plans. If you have received your mailing and have questions, please refer to the attached FAQ and/or call the Dependent Verification Center at 1.800.725.5810. The Dependent Verification Center will only speak to the employee and will not disclose information to spouses or child(ren) of an employee.
